George Levine's unease with critics who are unable or unwilling to account for their continuing preoccupation with literary texts they expose as "imperialist, sexist, homophobic and racist" illumines the contradiction fueling the reduction of aesthetics to ideology.1 Cultural studies that deploy (...) literature as evidence of the aesthetics' socio-historical substance mask literature's capacity to break open new perspectives on reality by assuming that literary works are politically complicit with the aesthetics' strategic "mystification of the status quo" (A&I, p. 3). Criticism's indifference to its philosophical presuppositions exacerbates the paradox of denouncing a body of works that constitute criticism's aesthetic and intellectual heritage. Yet, by overlooking the philosophical presuppositions that set music's autonomy against practical affairs, new musicology accedes to the schema it recoils against.The tradition nurtured by Gadamer's philosophical hermeneutics and Ricoeur's hermeneutic phenomenology represents a critical current whose significance has been overshadowed by postmodernist investments in decoding music's social and political content. For Ricoeur, imagination is productive when the fictions that works create affect our understanding of ourselves and our world by re-describing reality. Aesthetics' alignment with ideology encounters a limit in the power works evince by unfolding different ways of seeing or hearing reality. With this thought in mind I have turned over many volumes (...) of histories and collections of oracles, and with difficulty I happened upon one oracle said to be of the Sibyl of Erythrae or of Phaennis of Epirus. Nicomedes, the son of Prusias, put his confidence in this oracle, and interpreting it in an advantageous sense he took up war against his father, Prusias, at the persuasion of Attalus.’ Zosimus proceeds to quote twenty-one lines of hexameter verse, which have come down in a rather corrupt state, but of which the general sense is reasonably clear. Background: Clinical ethics support instruments aim to support healthcare professionals in dealing with moral challenges in clinical practice. CURA is a relatively new instrument tailored to the wishes and needs of healthcare professionals in palliative care, especially nurses. It aims to foster their moral resilience and moral competences. Aim: To investigate the effects of using CURA on healthcare professionals regarding their Moral Resilience and Moral Competences. Design: Single group pre-/post-test design with two questionnaires. Methods: Questionnaires used were the Rushton (...) Moral Resilience Scale measuring Moral Resilience and the Euro-MCD, measuring Moral Competences. Respondents mainly consisted of nurses and nurse assistants who used CURA in daily practice. Forty-seven respondents contributed to both pre- and post-test with 18 months between both tests. Analysis was done using descriptive statistics and Wilcoxon signed rank tests. This study followed the SQUIRE checklist. Ethical considerations: This study was approved by the Institutional Review Board of Amsterdam UMC. Informed consent was obtained from all respondents. Results: The total Moral Resilience score and the scores of two subscales of the RMRS, that is, Responses to Moral Adversity and Relational Integrity, increased significantly. All subscales of the Euro-MCD increased significantly at posttest. Using CURA more often did not lead to significant higher scores on most (sub) scales. Conclusion: This study indicates that CURA can be used to foster moral resilience and moral competences of healthcare professionals. CURA therefore is a promising instrument to support healthcare professionals in dealing with moral challenges in everyday practice. (shrink)

Directional deep brain stimulation contacts provide greater spatial flexibility for therapy than traditional ring-shaped electrodes, but little is known about longitudinal changes of impedance and orientation. We measured monopolar and bipolar impedance of DBS contacts in 31 patients who underwent unilateral subthalamic nucleus deep brain stimulation as part of a randomized study. At different follow-up visits, patients were assigned new stimulation configurations and impedance was measured. Additionally, we measured the orientation of the directional lead during surgery, immediately after surgery, and (...) 1 year later. Here we contrast impedances in directional versus ring contacts with respect to local anatomy, active stimulation contact, and over time. Directional contacts display larger impedances than ring contacts. Impedances generally increase slightly over the first year of therapy, save for a transient decrease immediately post-surgery under general anesthesia during pulse generator placement. Local impedances decrease at active stimulation sites, and contacts in closest proximity to internal capsule display higher impedances than other anatomic sites. DBS leads rotate slightly in the immediate postoperative period but otherwise remain stable over the following year. These data provide useful information for setting clinical stimulation parameters over time. (shrink)

ObjectiveTo prospectively study the cingulate cortex for the localization and role of the grasping action in humans during electrical stimulation of depth electrodes.MethodsAll the patients with intractable focal epilepsy and a depth electrode stereotactically placed in the cingulate cortex, as part of their pre-surgical epilepsy evaluation from 2015 to 2017, were included. Cortical stimulation was performed and examined for grasping actions. Post-implantation volumetric T1 MRIs were co-registered to determine the exact electrode position.ResultsFive patients exhibited contralateral grasping actions during electrical (...) stimulation. All patients had electrodes implanted in the ventral bank of the right cingulate sulcus adjacent to the vertical anterior commissure line. Stimulation of other electrodes in adjacent regions did not elicit grasping.ConclusionGrasping action elicited from a localized region in the mid-cingulate cortex directly supports the concept of the cingulate cortex being crucially involved in the grasping network. This opens an opportunity to explore this region with deep brain stimulation as a motor neuromodulation target for treatment in specific movement disorders or neurorehabilitation. (shrink)

This intervention study examined the effects of a career crafting training on physicians' perceptions of their job crafting behaviors, career self-management, and employability. A total of 154 physicians working in two hospitals in a large Dutch city were randomly assigned to a waitlist control group or an intervention group. Physicians in the intervention group received an accredited training on career crafting, including a mix of theory, self-reflection, and exercises. Participants developed four career crafting goals during the training, to work on (...) in the subsequent weeks, after which a coaching conversation took place over the phone. Physicians in the control group received no intervention. A pre- and post-test 8 weeks later measured changes in job crafting and career self-management and employability of 103 physicians that completed the pre- and post-test. RM ANOVAs showed that the intervention enhanced perceptions of career self-management and job crafting behavior to decrease hindering job demands. No support was found for the effect of the intervention on other types of job crafting and employability. This study offers novel insights into how career crafting can be enhanced through training, as this is the first empirical study to examine a career crafting intervention. HR managers can use the outcomes to develop tailored career policies and career development practices. (shrink)

PurposeThe purpose of this study is to describe the implementation and outcomes of an Eye Movement and Desensitization Reprocessing treatment-program for women with posttraumatic stress disorder after childbirth.MethodsA prospective cohort-study with pre- and post-measurements was carried out in the setting of an academic hospital in the Netherland. Included were women who gave birth to a living child at least 4 weeks ago, with a diagnosis of PTSD, or severe symptoms of PTSD combined with another psychiatric diagnosis. All received up (...) to 8 sessions of EMDR-therapy. The posttraumatic stress disorder Checklist for DSM-5 was administered before and after treatment. Trauma history was assessed before treatment with the Life Events Checklist for the DSM-5, the Childhood Trauma Questionnaire and the Childbirth Perception Scale. Descriptive statistics were used.ResultsForty-four women were referred, 26 met the inclusion criteria. After treatment, none of the women met the criteria for diagnosis of PTSD after on average 5 weekly sessions of EMDR- therapy. These outcomes are promising, as they were achieved in women with relatively high levels of psychiatric comorbidity and high rates of previous mental health treatment.ConclusionImplementing an EMDR-treatment program for women with PTSD after childbirth in the setting of an academic hospital is feasible and effective. Key factors for success include a close collaboration between the relevant hospital departments and a thorough case conceptualization addressing the etiology of the PTSD. (shrink)

We estimate that 208,000 deep brain stimulation (DBS) devices have been implanted to address neurological and neuropsychiatric disorders worldwide. DBS Think Tank presenters pooled data and determined that DBS expanded in its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. The DBS Think Tank was founded in 2012 providing a space where clinicians, engineers, researchers from industry and academia discuss current and emerging DBS technologies and logistical and ethical issues facing the field. (...) The emphasis is on cutting edge research and collaboration aimed to advance the DBS field. The Eighth Annual DBS Think Tank was held virtually on September 1 and 2, 2020 (Zoom Video Communications) due to restrictions related to the COVID-19 pandemic. The meeting focused on advances in: (1) optogenetics as a tool for comprehending neurobiology of diseases and on optogenetically-inspired DBS, (2) cutting edge of emerging DBS technologies, (3) ethical issues affecting DBS research and access to care, (4) neuromodulatory approaches for depression, (5) advancing novel hardware, software and imaging methodologies, (6) use of neurophysiological signals in adaptive neurostimulation, and (7) use of more advanced technologies to improve DBS clinical outcomes. There were 178 attendees who participated in a DBS Think Tank survey, which revealed the expansion of DBS into several indications such as obesity, post-traumatic stress disorder, addiction and Alzheimer’s disease. Some years ago, in the course of a general critique of what has sometimes been referred to as the covering law theory of explanation, I made the claim that perfectly satisfactory explanations can often be provided by indicating only one or a few necessary conditions, where we remain ignorant of the sufficient conditions, of what we nevertheless claim to understand. What seemed to me one identifiable type of such explanations I called “explaining how-possibly,” because it was a type more naturally (...) given in response to the question how it could be that a certain thing happened than to the more familiar question why it did so. In the present paper I propose to review certain objections to this thesis which have come to my attention in the interval, and to make such concessions as these seem to require. In fact, the concessions will be minor, since the general thesis still seems to me quite defensible. (shrink)

This study explores the impact of participation in a series of moral case deliberations (MCD) on the moral craftsmanship (MCS) of Dutch prison staff. Between 2017–2020, ten MCDs per team were implemented in three prisons (i.e., intervention group). In three other prisons (i.e., control group) no MCDs were implemented. We compared the intervention and control group using a self-developed questionnaire, administered before (pre-measurement) and after the series of MCDs (post-measurement). Results After the MCDs, participants scored significantly higher on 7 (...) of the 70 items related to MCS. On some items there were significant impact differences between the various professional disciplines. Discussion Possible explanations for a relatively low impact are discussed. A shorter and validated questionnaire is needed in order to further study the MCS of professionals and the impact of Ethics Support Services (ESS). Conclusions There was a positive development on some elements of MCS after participation in a series of MCDs. (shrink)

I propose that Professor Engelhardt is correct in his description of our present epoch, though partially but critically misguided in (...) his diagnosis of why, and thus falls short in a prescription for the restoration of salus. Her latest book, Wittgenstein’s Ladder: Poetic Language and the Strangeness of the Ordinary, provides an opportunity for reflection on a body of writing remarkable both in its consistency and its constant reinvention. Indeed, reading Perloff (...) through Wittgenstein’s Ladder throws her other work over the last twenty years into a new light, as many concepts crucial to her vocabulary—not only theory but also modernism, artifice, avant-garde—are here turned around and forced into new roles. To ask whether and to what extent Wittgenstein’s Ladder constitutes a break in her career is simultaneously to raise the question of critical and poetic theory.Theory Inside and Out“It is in this sense that grammar may be said to replace theory,” writes Marjorie Perloff in the introduction to Wittgenstein’s Ladder [18]. The sense in question is Ludwig Wittgenstein’s, or rather the sense of the late (post-1929) Wittgenstein inflected through Perloff’s post-poststructuralist sensibility. As the sentence preceding this remarkable claim puts it: “For although the later Wittgenstein was to argue that grammar is precisely the key to understanding a given proposition, that there is no essence above and beyond a specific grammatical structure, one nevertheless must ‘distrust’ grammar in the sense of interrogating it as stringently as possible” [17–18].Of course, formulations of theory as a style of distrust have been a commonplace in theoretical discourse for some time. It would not be difficult to argue that Perloff’s explicit advocacy of grammar through Wittgenstein—whose philosophy, she argues, “is indeed intentionally ‘antiphilosophical,’ its purpose being precisely to determine in what circumstances philosophy should be ‘against’ philosophy and why” [12]—is of a piece with recent attempts to think the limits of theory in theory itself. But such a reduction would seriously misread Perloff’s project. We would do well to question, for example, the obvious similarities between Perloff’s description of a duplicity in the project of grammar/theory and the model it might be taken as at once pointedly evoking and evading, Paul de Man’s now-classic essay “The Resistance to Theory.” Despite their apparent similarities, the startling claims of Wittgenstein’s Ladder have little in common with de Man’s suggestion that “resistance may be a built-in constituent of [theoretical] discourse,” and that “the polemical opposition, the systematic non-understanding and misrepresentation, the unsubstantial but eternally recurrent objections, are the displaced symptoms of a resistance inherent in the theoretical enterprise itself” [12].For one thing, de Man explicitly distances his conception of a theoretical project deeply threatening to the claims of knowledge from one rooted in concepts of grammar: [End Page 67][A]s long as it remains grounded in grammar, any theory of language, includ-ing a literary one, does not threaten what we hold to be the underlying principle of all cognitive and aesthetic linguistic systems. As long as it leaves this principle intact, there is nothing threatening about literary theory. The continuity between theory and phenomenalism is asserted and preserved by the system itself. Difficulties occur only when it is no longer possible to ignore the epistemological thrust of the rhetorical dimension of discourse, that is, when it is no longer possible to keep it in its place as a mere adjunct, a mere ornament within the semantic function.[14; emphasis added]In other words, de Man places grammar opposite the project of criticism, which he identifies with rhetoric and ultimately with reading. This paper deals with problems of quantum physics and, in particular, with the problem of scientific objects in quantum physics from the epistemological point of view previously defined in the author's books, A Philosophy (...) of Science and The Basis and Structure of Knowledge. The argument of the paper, however, has not been stated before in the direct and concise manner here attempted and may therefore contain elements of novelty even for those readers who are familiar with the books just mentioned. Historians generally see ideas as the product of circumstances, looking beyond the idea to the external factor which influenced its acceptance. Behind an idea there are acknowledged or, more commonly, unacknowledged clusters of assumptions shared by a social group. Although these clusters influence thoughts, they cannot be traced as direct causal agents. In the connection between situations and ideas, how the situation is perceived is more important than what is objectively true. Rough causal laws can be outlined by correlating types (...) of social conditions with types of states of mind. Sets of ideas point beyond themselves to the background against which they were framed. The following chapter treats of the philosophy of "actual" idealism and compares the views of Kant and Gentile on relations between moral conscience and freedom. According to Yalentini, Gentile's idealism is essentially an ethical view. This chapter concludes with noting (...) that Gentile 's solution to the problem of historical knowledge differs profoundly from that of Hegel and Croce. For "reason" is not, for Gentile, a particular interpretation of historical process, but is rather the "reason" of consciousness in activity. In Chapter In, Valentini discusses Gentile's views on art and religion. The section on art concludes with a comparison between Gentile on feeling and Kant on the transcendental unity of apperception. Whereas art is supposed to be one abstract moment of spirit, religion is the other abstract moment. Valentini suggests that Gentile and Feuerbach hold similar religious views on the attributes of divinity, which are nothing other than the loftiest qualities which man feels to possess in his essence. The history of religion is then the history of the progressive humanization of God. Gentile's religion represents a sort of modem Catholicism in that it understands revelation as a fact of conscience, church as a historical institution, dogma as having a negative character and God as a member of the dialogue between God and man. Moreover, Gentile's Catholicism has a national character; it is the religion of the Italian people. Chapter Iv treats of the relations between individual and state. For, according to Gentile, an individual is inconceivableapart from the social relations which lie within him. Here Valentini distinguishes in detail the differences and similarities between the views of Hegel and Gentile on the relations between state and individual. The differences between the views of these two philosophers are due to the differences between their methods. The method of Hegel is empirical and historical, and examines the state (which remains historical) from an external point of view. According to an idealistic method, however, thought is not to be the spectator of a process, but in every instance, an actor, and philosophizing does not result from an analysis of historical reality, but from the negation of this reality in the name of an ideal, which philosophy is to draw from it (reality). The Appendix includes a critical evaluation of the theory of the dialectic. Here Valentini hopes to distinguish between the imaginary and the real or historical applications of the dialectic. He begins his evaluation by asking: Does the doctrine of the dialectic, like the philosophy of idealism, possess negative connotations, which cannot be separated from it? Valentini suggests that in order to answer this question, we must determine the extent to which the dialectical scheme is constitutive of every concept or truth in general. For this is the way in which Hegel intended that the dialectic be understood and evaluated. Some views in Hegel's philosophy of nature are then criticized according to empirical criteria. In conclusion, Valentini attempts to answer both "internal" and "external" criticisms of Marx's view of the dialectic, and shows that there are problems involved in its application, which still have to be resolved. One is the fact that Iso Kern was able to make extensive use of hitherto unpublished materials (manuscripts, lecture notes, letters, etc.), available only in the Husserl Archives and here quoted for the first time--and 98 HISTORY OF PHILOSOPHY quoted in great numbers.
